Hconnect attends the `Showcasing Community Services for Frail Patients` Event at Chesterfield Royal Hospital to help develop healthcare professionals understanding of Carers needs

 

Staff members Claire Travis and Tina Curran from the Hconnect Project attended the `Showcasing Community Services for Our Frail Patients` which took place at the Chesterfield Royal Hospital on Thursday 18th September.  This drop-in event was aimed at all NHS Healthcare Professionals in a bid to help better inform them of the help and support available locally to Frail Patients and their Carers.

Over 35 NHS staff members attended the event and members of the Hconnect team were happy to chat about the Call for Carers learning and development programme, our work with Pilot sites, as well as letting them know about the general Carers Awareness Training which is available to them for free.

These are available to book on the DAACSS website by clicking on the Hospital Discharge Liaison Project tab and then clicking the`Training Hconnect` tab. The General Carers Awareness training sessions are all completely free and are there to further develop all Healthcare professionals knowledge and understating linked to identifying carers earlier on, and aims to give them the knowledge and confidence to be able to better support and signpost Carers to get the support that they might need in order to continue with their Caring role.
