Staff members from Derbyshire All Age Carers Support Services and the ARF Hconnect Project attended the  popular Carers Event at St George`s Hotel, Derby on Friday 2nd May.

This event provided Carers with a wealth of information and support linked to their Caring role, as well as providing them with the opportunity to have their say on the current Hconnect Project running within 6 pilot sites in Derbyshire and Derby City.

Carers learned about the Hconnect Project which aims to improve Carers experiences of the Hospital Discharge Process linked to their loved ones, this is done through training and supporting NHS professionals to better understand Carers needs and help signpost Carers to supporting charities and organisations.

This event also allowed Hconnect to network with other healthcare professionals to enable valuable links to be made to help Carers and their loved ones.
