Referral Guidance – What Happens Next
Making a referral to the Derby Carers Information, Advice and Support Service
When you complete a referral, you will be registered with our service as someone who provides unpaid support to a family member, partner, friend, neighbour or adult child.
Once registered, you will receive regular updates about groups, activities, events and support available in Derby City.
Our aim is to make the process simple, clear and supportive from the very beginning.
What happens after you make a referral (could this be done as a fancy pathway?)
Here is the pathway you can expect:
- Referral submitted
You complete the online referral form.
- Registration
We add your details to our secure system so we can keep in touch and offer support.
- Welcome pack sent
You will receive a welcome pack with information about the service and what we offer.
- Contact from our team (if requested)
If you have asked us to get in touch, a Carer Support Worker will contact you using your preferred method (phone, email or text).
- Information, advice and support
Your Carer Support Worker will:
- Listen to your situation
- Offer information and advice
- Help you identify what support may help you in your caring role
- Connect you with groups, activities or services that may benefit you
- You may need a follow‑up appointment, or it might be more helpful to meet in person. If so, we’ll arrange the next appointment for you.
We are here to support you at your pace and in a way that works for you.
Information we need at the referral stage
To help us understand your situation and offer the right support, we ask for:
About you (the person providing support)
- Name
- Address
- Date of birth
- Ethnicity
- Email address
- Telephone number
- Your health and wellbeing, and the health of the person you care for
- Any risks we should be aware of
- Communication preferences or difficulties
- The area the person you care for lives
- Your relationship to them

We really value the support you provide to carers, and we want to make sure we can follow up quickly and offer the right help at the right time. To do this, we kindly ask that all professionals use our online referral form and provide as much of the requested information as possible.
